We believe the use of psychedelics is a pathway to self-actualization. Psychedelic Performance Optimization Coaching is both looking at traumas, disorders, anxiety, depression, PTSD, etc. – as well as looking at strengths, gifts, and empowered pieces of who you are. All humans are perfectly imperfect. We believe all humans possess higher healing wisdom within themselves. This medicine helps us contact that higher healing intelligence to unlock problematic patterns and access helpful ways of being, which lead us to be our better selves. We believe you are not one fixed and stagnant self; you are constantly evolving. You are only a series of patterns of behaviors – if you play tennis regularly, this activity makes you a tennis player. If someone has an identity and has or lacks the behavior or action needed to be a thing – perhaps that analysis is inaccurate. For example, the term depressed is linked to a series of behaviors of depressed activities or doings, such as unable to get up, lack of appetite, staying in the dark, social isolation. Whereas thriving is a series of behaviors, such as exercising, eating well, having an active social life, etc. Our goal is to aid you in the process of helping you find the process of becoming your best self by doing the work given to you through the insights of this medicine. The medicine will show you who you are capable of being. We will create an integration plan to make the work “real” in your life. After several days, weeks, or months you will come in again with this new sense of self – and see what new heights are possible now that you have done the work to integrate the healing wisdom imparted with the medicine.

What is Psychedelic Integration Coaching?

Psychedelic integration coaching is a specialized form of personal coaching that focuses on supporting individuals who have had psychedelic experiences in integrating and making meaning of their insights and discoveries. Psychedelic substances such as psilocybin mushrooms, LSD, or ayahuasca can induce profound and transformative experiences that can have lasting effects on an individual’s perception, beliefs, and sense of self. Integration coaching recognizes the importance of providing guidance and support during the integration process to help individuals integrate these experiences into their daily lives healthily and constructively. A psychedelic integration coach works closely with clients to explore and unpack the insights, emotions, and challenges that arise from their psychedelic journeys. The coach provides a safe and non-judgmental space for clients to share their experiences and helps them navigate the sometimes overwhelming or confusing aftermath of a psychedelic experience. Integration coaching may involve discussions about the client’s intentions for the journey, the emotions and thoughts that emerged during the experience, and the implications and meaning of the insights gained. The coach helps clients integrate these newfound perspectives into their relationships, work, personal goals, and overall well-being. The ultimate goal of psychedelic integration coaching is to facilitate the client’s growth, self-awareness, and personal transformation as they incorporate the lessons and revelations from their psychedelic experiences into their everyday lives.

Difference between Coaching and Therapy

Therapy and personal coaching are valuable tools for personal development and growth, but they differ in their approaches, goals, and focus areas. Therapy primarily focuses on addressing mental health issues, emotional well-being, and resolving past traumas. It aims to provide individuals with a safe space to explore their thoughts and feelings, gain insight into their behaviors and patterns, and develop coping strategies. Therapists are trained professionals who often use evidence-based techniques and theories to help clients overcome psychological challenges. On the other hand, personal coaching is more future-oriented and goal-driven. Coaches work with individuals to help them identify their strengths, set specific goals, and develop strategies to achieve them. Coaches provide support, guidance, and accountability to help clients unlock their potential, overcome obstacles, and achieve desired outcomes. Unlike therapy, coaching does not diagnose or treat mental health conditions but instead focuses on enhancing performance and personal satisfaction. Another distinction between therapy and personal coaching lies in the nature of the client-coach or client-therapist relationship. Therapy typically involves a therapeutic alliance where the therapist holds a position of expertise and authority. Therapists maintain professional boundaries and adhere to strict ethical guidelines to ensure the well-being and confidentiality of their clients. In coaching, the relationship is more collaborative, with the coach acting as a partner or facilitator. Coaches often bring their expertise, experiences, and perspectives to the table but ultimately empower clients to take ownership of their actions and decisions. In summary, therapy and personal coaching differ in their focus, goals, and the nature of the client-practitioner relationship. Therapy is more oriented toward addressing mental health concerns, resolving past traumas, and promoting emotional well-being. Personal coaching, on the other hand, emphasizes future-oriented goal setting, personal development, and performance enhancement. Ketamine-Assisted Integration Coaching

Currently, we work with the legal psychedelic, Ketamine. In the future, MDMA is slated to go through Phase 3 trials and may be legal or accessible via prescription in late 2023 or early 2024. Psilocybin is also in Phase 3 Trials and may be legalized mid to late 2024.

KAP:  KETAMINE-ASSISTED INTEGRATION COACHING INFORMATION

Please read and review the following form carefully prior to your initial KAIC appointment and bring any questions you have to the attention of your physician and/or therapist. 

WHAT IS KETAMINE?

Ketamine is a Schedule 3 drug approved by the FDA since 1970.  Unlike Schedule 1 substances such as psilocybin, LSD, and MDMA, which were banned in the Drug War and are working their way back from the black-market step by step, ketamine was always a legal pharmaceutical.  Thus, it can be prescribed “off-label” for psychological conditions such as PTSD, Depression, Anxiety, Addiction, Suicidal Ideation, Obsessive Thought Patterns, and more.

Ketamine is a unique drug that has a wide range of effects: 

In relatively small amounts, Ketamine is increasingly found as a party drug, although used by less than 1% of the population in the U.S.  This carries the risks of any illegal white powder, including unknown purity and uncertain dosage with consequently unpredictable results.  Under these conditions, its use can become compulsive and unhealthy for some people. Large dosage results in total unconsciousness as a dissociative anesthetic used worldwide in medical settings.  Ketamine became known as the “buddy drug” on the battlefield because it could be administered quickly and safely under difficult circumstances.  However, as the drug wears off, disorientation while returning to normal consciousness can be an unwanted side effect.  Children seem to handle this better (“You might have a strange dream…”).  Ketamine is also common in veterinary use. In a middle range, usually with dosages of .5- 1.0 mg/kg IV or IM, or 100-300 mg in Rapidly Dissolving Lozenge form, ketamine has been referred to as a “situational psychedelic”.  The qualities of the experience are greatly affected by factors such as intention, support, music, the aesthetics of the environment, physical comfort, and a sense of safety to let go and explore the inner world.  

HOW DOES KETAMINE WORK?

Our current understanding is that Ketamine’s mechanism action involves modulation of the neurotransmitter Glutamate and is achieved by affecting the NMDA receptor. Ketamine also has effects on the GABA, Dopamine, and monoaminergic pathways. Research supports the effects of increased neuroplasticity as well. Ketamine offers unique modes of action, unlike other psychiatric drugs such as SSRIs, SNRIs, antipsychotics, and benzodiazepines.  Recently a patentable variation of ketamine, Esketamine, known by its trade name as Spravato, was researched and approved by the FDA for addressing treatment-resistant depression. Some insurance covers the cost of around $800 per dose. However, there is no evidence that it works any better (and may not be as effective) than regular generic ketamine at under $5 for the same quantity. Psychiatric use of ketamine has become widespread in recent years and has been studied and promoted by researchers at the National Institute of Mental Health. We look forward to having more clinical data about this remarkable chemical to guide our practice.

KETAMINE-ASSISTED INTEGRATION COACHING (KAIC) 

Ketamine-Assisted Integration Coaching KAIC uses ketamine in one or more sessions integrated with psychotherapy.  In this practice, the beneficial qualities of KAP come from a combination of several factors:

*Biochemical effects that may include a decrease of depression.

*Shifts in consciousness during a KAIC session leading to insight and new perspectives.

*Increase of neuroplasticity that allows for change during the period after treatment.

Although some patients have experienced relief from depression from receiving IV ketamine infusion as a purely biochemical treatment, we have found that its full value is as an adjunct to psychotherapy.  Therefore, we provide Ketamine Assisted Psychotherapy that includes preparation, support during the actual ketamine session, and integration in the hours, days, and weeks after sessions.  

WHAT TO EXPECT DURING KETAMINE-ASSISTED INTEGRATION COACHING (KAIC)?

A crucial component of KAIC is preparation and integration before and after a KAIC session.  Our programs allow time for these sessions with your therapist.  KAIC dosage and protocols vary depending on the individual’s needs and response to the medication. This will be discussed in person as it will be unique to everyone.  

Although ketamine may be administered by intravenous, intramuscular, subcutaneous, and intranasal/intra-nasal routes, in our office, we are focusing on the sublingual/buccal (under the tongue or in contact with the cheek) route that does not require needles.  

Some conditions create risk factors for participation in KAIC, and a detailed medical assessment is required to ensure your safety.  Rapidly Dissolving Lozenges may be prescribed and picked up at a compounding pharmacy following this medical evaluation.  

In a KAIC session, the RDLs are placed under your tongue.  After they dissolve, saliva is held without swallowing for about 10-15 minutes.  At that point, you may swallow or have the option to spit it out (most people choose to swallow, which extends the altered state).  By this time, the initial effects are usually felt, and then build over the next 10-20 minutes.  The intensity usually wanes after 60-90 minutes but can linger for another hour or more, especially with higher doses. 

The overall duration of a KAIC session is usually about 3 hours in length, including preparation and integration at the end.  In our center, clients are encouraged to take plenty of time to ensure they feel fully complete.  If you no longer need or want the therapist’s attention, you can continue to relax with music or leave the session room as long as safety agreements are met.

Clients will need to arrange for transportation after a KAIC session since there is to be no driving for the rest of the day.

Psychedelic-assisted integration coaching has a long history of success when conducted under-skilled and ethical conditions.  However, there also is no guarantee regarding the outcome.  It is completely normal to feel some apprehension at stepping into the unknown.  At the same time, this adventure carries the potential for positive changes that we are seeking and even possibilities beyond our imagination.

Ketamine can potentially create a unique and sometimes profound non-ordinary state of consciousness (NOSC).  A wide variety of effects may be experienced, depending on dosage, intention, the psychological make-up of the individual, and factors that may be termed spiritual or mystical.  You will have a chance to discuss this range of possibilities with your therapist. 

If unsettling or challenging experiences arise at any time, your therapist will be there for you.  They will always be nearby in the room, giving you their attention (not checking their phones!) and intention for your beneficial results.  Since ketamine tends to be an internal experience, there is no physical touch unless it is requested.  Sometimes a hand to hold or a comforting touch on the shoulder is reassuring during a period of disorientation, or the client would like the therapist to sit nearby, and we are happy to provide happy provide this.  At the same time, having many years of experience with trauma therapy and psychedelics, we trust that clients can rely upon their own Inner Healing Intelligence as a guide on the journey.

As in any standard ethical psychotherapy, sexual touch or sexual interactions of any kind between the therapist and client are not permitted.  This is especially important when altered states are involved.  Clients may have their own sexual feelings or issues around sexuality come up, of course, and these may be important and beneficial when accepted and allowed to unfold in a safe and supported space.  

PERSONALIZED SERVICES

We operate in an individualized, “out of the box” model.  Our center is a home office with unusual aesthetics, not the usual clinical setting.  The artwork and objects here are designed to support an environment of transformation.  You may notice images from various traditions, although they are not intended to prefer or exclude any particular religion or culture.  We acknowledge that religion has been a source of trauma and inspiration.  The bottom line is that each person’s spiritual belief (or non-belief) and practice is honored and welcomed on their path of healing and awakening.  

There are no Shamans here, and we don’t call ourselves Healers.  Rather, we are facilitators who help set up the conditions where healing can happen.  As an option, some people resonate with a sense of ceremony that we’re happy to provide.  Elements such as choosing an “oracle card” from a deck, burning sage, or offering a prayer can enhance the ketamine experience, depending on personal preferences.

RELATIONSHIPS

In this work, we don’t keep an artificial distance between professionals and those seeking help; we acknowledge our shared humanity on life’s journey.  While doing this deep, transformative work, therapists and clients will likely develop a sense of alliance and possibly friendship.  This is not a problem as long as it is understood that the intention and the primary focus of our work together is always for the client’s benefit.  If there are ever any questions about relationship limits, please bring them up.  Your sense of safety is paramount in doing this work.  Learning to speak up for your own comfort or boundaries can be an important area of growth.  

BENEFITS:

Potential benefits resulting from KAIC may include: 

Improved understanding and sense of self, promotion of novel perspectives and outlooks on life’s challenges, decreased anxiety/depression, decreased obsessive/addictive patterns, improved resilience, resolution of trauma, increased desire to engage in life and interpersonal relationships, enhanced compassion for self and others, the development of healthy patterns, improve one’s outlook on the future.

Potential Adverse Events:

Ketamine has an extensive safety record, and there are potential side effects. These may include Lightheadedness/dizziness, blurred/altered/double vision, slurred speech, mental confusion, excitability/agitation, changes in hearing and tactile senses, nausea/vomiting, anxiety, the feeling of floating or falling, and decreased awareness of bodily functions.

Ketamine does cause an elevation in blood pressure, much like it occurs with exercise. We will have a blood pressure cuff and pulse oximeter to monitor your vital signs signed if needed. We will have medications available if needed and with the client’s consent for severe elevation in blood pressure.

Nausea and vomiting are rare, and it is for this reason that we request no oral intake, except for water, for 4 hours prior to your session. We do have medication available to treat these symptoms should they occur.  

Urinary Tract Problems:
Abuse and repeated high-dose use of ketamine have been linked to urinary tract symptoms and even permanent bladder dysfunction. This is believed not to occur within the framework of treatment programs under the supervision of a physician. 

Abuse Potential:

The medical evidence does demonstrate abuse potential. For this reason, it is only to be used under the supervision of a therapist/medical professional team.

You will be assessed for safety prior to leaving the office. You may not drive after a KAP session; please make arrangements for transportation.

ALTERNATIVES TO KAIC:

Inclusive Activism may include a wide range of methods that are effective with or without the use of ketamine.  We have over 30 years of experience and will be happy to discuss options and create a personalized program together to meet your needs.

SAFETY PLAN FOR KAP (KETAMINE ASSISTED INTEGRATION COACHING): 

Preparation and Integration are essential parts of a safety plan.

Agreements

The client agrees to:

  1. Read instructions for preparation for KAIC and get satisfactory answers to all questions. 
  2. Abstain from food or drink (other than water) for three hours prior to your KAP session. 
  3. Follow any direct instructions given to you by your clinician until it is determined that the session is over. 
  4. Remain at the session’s location until the clinician decides you are ready to leave the clinic. 
  5. Do not drive after your ketamine session. If leaving from the clinic, you must have pre-arranged transportation and cannot leave via walking, bicycle, etc. You will also agree not to drive or operate heavy machinery for a minimum of six (6) hours after your session is finished. 

Basic Preparation includes:

-Informed Consent

-A trained therapist to be present throughout the KAP session.

-Professional Medical Evaluation to determine if KAP is considered safe and beneficial and to identify risk factors that would exclude the client from participating in KAP.

-Vital signs will be monitored as indicated.

-Equipment is in the room should the client experience respiratory distress or extreme sedation: An oxygen tank with a nasal cannula and an Ambu bag for assisted manual ventilation.

-Epinephrine in the form of an “Epi-Pen” is available should there be an anaphylactic-type reaction to Ketamine which is rare.

-Other basic medications are available, i.e., Diphenhydramine (Benadryl) should there be a mild allergic reaction; Ondansetron (Zofran) should the client experience nausea/vomiting; Tylenol and Ibuprofen as needed for pain; SL Nitroglycerin for extreme hypertension; Baby Aspirin for symptoms concerning for cardiac ischemia and lastly Diazepam for severe agitation, anxiety or dysphoria unresponsive to therapeutic interventions. 

In the event of dire medical or psychiatric distress, 911 would be called.

Your identification and medical information are legally protected by HIPAA (Health Insurance Portability and Accountability Act passed by Congress in 1996- further information can be found at HHS.GOV). Your information will be kept confidential on HIPAA-protected, encrypted programs. If you wish to have others access these records, you must sign a release of protected information form. We collect clinical data from your sessions that may be published in clinical literature without identifying information. The purpose of collecting this data is to further the research in ketamine-assisted integration coaching and help others. If your emergency contact person(s) or emergency services are contacted without your permission, the least amount of information necessary to ensure your safety will be disclosed.

Cannabis-assisted coaching refers to a coaching approach that incorporates the intentional and regulated use of cannabis as a tool to support personal growth, self-discovery, and transformation. This coaching modality can be particularly beneficial for individuals seeking alternative approaches to self-exploration, creativity enhancement, stress reduction, or finding new perspectives on personal challenges.

The future of other psychedelic coaching medicines

The future of psychedelic-assisted coaching with substances like psilocybin and MDMA is a topic that is currently being explored through rigorous research and clinical studies. These studies provide valuable insights into these substances’ potential therapeutic benefits and applications in conjunction with coaching. Research on psilocybin-assisted therapy has shown promising results in treating various mental health conditions. Studies conducted at reputable institutions, such as Johns Hopkins University and Imperial College London, have significantly reduced depression and anxiety symptoms in participants who received psilocybin-assisted therapy. While promising, these substances require careful administration and professional guidance to ensure safety and maximize therapeutic benefits.
